If you have a child with a diagnosed or undiagnosed rare disease simply complete the following form and we will send you your FREE face mask. We are a small team working like busy bees (or should that be zebras!) to create these masks for our children, affected by rare and undiagnosed conditions. With this in mind we can only send one free mask per affected child per household.
We understand that many people are struggling at the moment which is why we are trying to help in any way we can.
